Professional referrals to Wigan Council Adult Social Care for care needs assessments, support planning, and adult social services.
⚠️ Important Update
New Referral Form Coming Soon
Adult Social Care is introducing a new Professional Referral Form. Continue using standard referral pathways until further notice. A follow-up email will be sent when the new form is live.
Key changes:
- New online Professional Referral Form will be available on the council website
- Referrals will no longer be accepted via email or post
- All professional referrals must be submitted through the online form

Consider Adult Social Care referral for:
