Team Structure

CRT is made up of several specialist arms:


🏠 Hospital at Home

A dedicated multi-disciplinary team consisting of Occupational Therapists (OT), Physiotherapists and Nurses providing rapid, same day holistic care in a community setting.

HOIS provides care to the most vulnerable communities in society, supporting both individuals and families.

ACPs can assess, diagnose and treat a variety of presentations to help keep patients at home and reduce hospital admissions.

🩸 Home Blood Transfusion Team

Hours

Monday - Friday, 08:00 - 17:00

A small team of experienced nurses carrying out home transfusions for patients who cannot attend PIU at the RAEI.

Criteria

  • Patient must be under a consultant
  • Referrals generated from the hospital

  • Whole blood transfusions
  • Platelet transfusions
